REGULATIONS FOR THE PROVISION OF HAIRDRESSER SERVICES
IN HOLLYWOOD HAIR SALONS
LEAD BY KLAUDIA DUSZYŃSKA

§1
GENERAL PROVISIONS


1. The Regulations define the rights and obligations of persons using hairdressing services, including:
hair extension and thickening services provided by the Hollywood Salon chain

2. The terms used in the Regulations mean, respectively:
(a) Agreement – ​​an agreement with annexes on the basis of which the Service Provider
provides hairdressing services to clients, including services in the field of
hair extensions and thickening. The Regulations constitute an integral part of the Agreement,
(b) Service Provider - Klaudia Duszyńska running a business under the name
Hollywood Hair Klaudia Duszyńska at the following address: ul. Belwederska 40/3, 00-594
Warsaw, NIP: 5060093284, REGON 147365472.
(c) Client - a natural person who is over 18 years of age and has full legal capacity

(d) HH Salon – means the Hollywood Hair hairdressing salon run by

3. In order to exercise the rights specified in the Regulations, the Customer may contact

4. Addresses of Hollywood Hair Salons and information about opening hours of HH Salons
are available on the Website.
5. HH Salons provide hairdressing services, including in particular treatments

hair coloring and styling. Depending on the location of the HH Salon, the range of services offered

6. The Regulations are made available to Customers free of charge via the Website.

§2
RULES FOR PROVIDING SERVICES


1. The services of the HH Salon may be used by adults who have full legal capacity.

(c) explanation of the technique and method of performing the service, and the duration of its duration

(d) discussion of contraindications to the performance of the service, and in the case of the service
hair extensions in circumstances that make it impossible to perform the procedure

(e) discussion of the rules of conduct after the service, including the rules of care and
styling, especially hair extensions, necessary to obtain
expected results of the service.
5. Before starting the service at the HH Salon, the Client selects the strands
hair from the offer available in the Salon, according to personal preferences. The service is
carried out after the customer's acceptance of the type and weight of the bands he has chosen
hair, selected color, their structure, thickness, degree of brittleness and dryness,
including their possible susceptibility to spinning or not and the acceptance of the due
remuneration for the service (cost of hair strands and fees for the service).
6. Detailed information on contraindications to the service and possible
side effects of hair extensions that may or may not occur, rules
procedures after the service is performed, including care and styling rules in particular
hair extensions regardless of the above information being provided to the Client
before the service is performed at the HH Salon, constitute an annex to the Agreement for the performance of
hair extension services and are available on the Website.
7. As part of the hair extension and thickening services provided, the Service Provider offers:

8. The free consultation service is dedicated to new clients who use the service for the first time.

9. As part of the follow-up visit, the Service Provider offers to Customers who have previously
used the hair extension or thickening services at the HH Salon, support and

10. The client using the services of the HH Salon is obliged to:
(a) read the Regulations and comply with their provisions, and

(b) booking visits to the HH Salon in accordance with the principles described in § 3 of the Regulations,
(c) inform the Service Provider about the existence of contraindications to the provision of the service
and other circumstances related to the Client's health condition,
natural hair and scalp condition that prevent the service from being performed
or which may influence its result,
(d) compliance with the accepted principles of social coexistence and norms of good
behavior and respect towards other people staying on the premises

10. On the premises of the HH Salon it is strictly forbidden to:
(a) smoking tobacco or e-cigarettes,
(b) possession and consumption of alcohol, narcotics and drugs,
(c) coming to appointments while intoxicated, intoxicated or under the influence of

(d) recording images and sounds without the prior consent of the Service Provider,
11. The client is obliged to arrive on time for the booked appointment. In the event of
delays exceeding 45 minutes, the Service Provider is entitled to refuse to perform
services. Such a delay will be treated as failure to cancel the visit on the date mentioned
referred to in § 3 section 8 with the consequences indicated in § 3 section 11.
12. The Service Provider, after obtaining the prior consent of the Customer, may record using
devices recording image and sound, the process of performing the service and the condition of the hair

§ 3
APPOINTMENT BOOKINGS


1. Appointments are booked at HH Salons. The client can book an appointment at
HH Salon as follows:
(a) in person at the selected HH Salon,

(d) electronically via the contact form available on the Website

(e) electronically by sending a private message via

4. The deposit can be paid in cash or non-cash at the HH Salon in

using the mobile payment system under the service mark "BLIK".
5. In case of reservations made by telephone or electronically, payments from
the deposit is made in the form of a transfer to the Service Provider's bank account
or by using the form enabling payment of the amount due

6. The Service Provider will inform the Customer about the appropriate number for paying the deposit
bank account electronically to the e-mail address provided by the Client or
via SMS message sent to the telephone number provided by the Customer,
depending on the arrangements between the Parties. In such a case, the deposit should be paid in

7. In the event of failure to pay the deposit in accordance with the provisions of this paragraph,

8. The client can change the date or cancel the booked visit free of charge.

11. The Service Provider is entitled to retain the deposit referred to in paragraph 3 in
case:
(a) the Client's failure to appear at a scheduled visit without prior notice

§4
PRICE LIST. PAYMENTS


1. The fees for using the services provided in HH Salons are specified in the price list,
which constitutes Annex No. 2 to these Regulations. The price list is available at
each of the HH Salons and on the Website.
2. The Service Provider may introduce periodic promotions, gift vouchers and
offer discounts to customers. Promotions and discounts cannot be combined. Information about the rules and
the periods of validity of the above-mentioned promotional campaigns will be regulated in a separate
regulations.
3. The following methods of payment for services are available at the HH Salon:
(a) in cash at the HH Salon,
(b) in non-cash form,
(c) in a non-cash form using the PayU installment system.
4. If you choose cashless payment, the payment is made in
in the form of an instant transfer or using a mobile payment system
under the service mark "BLIK". When making payments by bank transfer, the details

5. In the case of payment using the PayU installment system, the Service Provider
will commence the service once the funds have been credited to the Service Provider's bank account

§ 5
COMPLAINTS


1. The Contractor is responsible for non-performance or improper performance of services
provided in HH Salons in accordance with generally applicable legal provisions.
2. Any complaints regarding the services provided in HH Salons may be submitted
by the Client:
(a) in writing by leaving a letter at the HH Salon where the service is provided
has been implemented,
(b) by letter (registered mail) to the address of the HH Salon where the service was provided

(c) by mail (registered mail) to the following address: ul. Belwederska 44, 00-594

3. The Customer's complaint should include the name and surname, description of the issues (along with
photographs showing the condition of the hair) or the Client's requests and data

4. The Service Provider reserves that in order to respond reliably to the notification
complaint, the Client may be asked to come to the HH Salon in order to
inspect the condition of the client's hair strands and verify the causes

5. The response to the complaint will be provided in writing to
the indicated correspondence address or electronically to the indicated e-mail address
within 14 days of its receipt.
6. After completing the complaint procedure, the Customer may use the following options:
out-of-court methods of handling complaints and pursuing claims:
(a) apply to the provincial inspector of the Trade Inspection for
initiation of proceedings for the out-of-court resolution of the dispute,
(b) refer the case to the permanent arbitration court at the provincial inspector
Trade Inspection,
(c) contact the municipal or district consumer ombudsman or
a social organization whose statutory tasks include protection
consumers, including the Consumer Federation.
7. Additional information regarding the possibility of using the service by the Client who is

8. Using the above methods of handling complaints and pursuing claims

§ 6
PERSONAL DATA

§ 7
Final provisions
